To order prescriptions, make appointents or have access to areas of your medical records you should come into surgery to collect the application form and information leaflet. You will need to bring with you two forms of identification one of which should be photographic.
The process for on line appointments and prescriptions is straight forward however for access to medical records there are additional criteria which we are required to set, more information will be provided in surgery when this request is made.
Once we have your application form and have verified your Identification we will send you an access code to enable you to enter the system here
Do you need to re-order your medication? We are now using Electronic Prescribing which is a link between your pharmacy and the GP. This reduces your need to call into surgery and makes the process much quicker for you providing no queriey exists with your request. Some pharmacies are happy to deliver your medication to you. For more information and to register for this process speak with the pharmacy of your choice and they will register you.

We have had a number of requests for GPs to sign passport photographs to verify identity. Unfortunately we are unable to do this until at least December 2017 as the rules applying require a GP to have known a patient for two years before they can verify. This restriction does not apply to passports for children under two years old. Please note there is a fee of £25.00.
